Network Overview

Prevea Health Network is a preferred provider organization (PPO) consisting of physicians, hospitals, clinics and other health care professionals throughout Wisconsin. Insurance companies, TPA's, employer groups and other PPO networks place PHN within the various health insurance benefit plans they are insuring or administrating.

PHN has two panels of providers available:

Prevea Health Network Only

The PHN-only network includes providers associated with Prevea Health (formerly Prevea Clinic), St. Mary's Hospital Medical Center, St. Vincent Hospital, as well as independent medical professionals throughout Wisconsin.

Network Health Prevea PPO

Network Health Prevea PPO is referred to as the "blended" network. It includes the providers of PHN and Network Health Plan of the Fox Cities. Network Health Plan's panel includes providers from Affinity Medical Group, St. Elizabeth's Hospital, Mercy Medical Center, Chilton Medical Center, and other independent medical professionals throughout Wisconsin.
